Please complete this appointment request form each time you would like to visit the PVD CAP Food Pantry. This form helps our team schedule your visit, prepare for your appointment, and understand your household’s current needs.

If this is your first time requesting support from the Food Pantry, please complete the initial Food Pantry Intake Form before submitting an appointment request. After your intake form has been submitted and reviewed, you may use this appointment form for future visits.

Food Pantry appointments and items are available based on schedule availability and current inventory.
