Case Management is a supportive process that helps eligible Providence residents plan, seek, and secure services from PVD CAP and other community programs. Since every household's needs are different, case managers provide individualized guidance based on each person's strengths, challenges, and goals.

Connected Support Programs
Some PVD CAP services connected to Case Management require a referral from another program or agency. These programs are not generally open for direct public enrollment, but they play an important role in helping eligible residents access deeper support.
RI Works
RI Works provides intensive case management for eligible families referred through the Rhode Island Department of Human Services. The program supports parents and families as they work toward employment, self-sufficiency, and long-term stability.

The S.O.S. Program supports eligible LIHEAP clients who may need additional guidance and resources beyond fuel or energy assistance. The goal of the program is to help households work toward greater self-sufficiency and eventually reduce the need for ongoing LIHEAP support.

•Were referred through DHS, LIHEAP, or another service provider
Please note some programs require eligibility screening or a referral from another agency or PVD CAP department.
What to Expect
When you reach out or are referred to Case Management, a case manager may help assess your situation, identify strengths and barriers, and determine which programs or resources may be appropriate. From there, the team can help develop or update an action plan and connect you with internal or external services when available.
